All new patient appointments are conducted by video via Doxy.Me. All new patient paperwork must be completed prior to setting your appointment.
A follow-up appointment will be required 2-4 weeks after your initial appointment to become established.
Authorization for release of information to/from the office of JoEllen Butler.
In order to keep our cost down, we are strictly self-pay and do not accept insurance of any kind. However, we can provide instructions that may be helpful in potentially obtaining reimbursement from your insurance company.
- If you need prescription refills, please check with your pharmacy first.
- Ensure refill requests are being made during normal business hours and allow 24 hours for refills to be completed.
- Non-emergency refill requests made during non-business hours will be completed the following business day. (If a request is made on Friday, the request may not be completed until Monday morning). It is your responsibility to make sure that your prescriptions are filled on time and during business hours.
- We require 24 hour notice to cancel/reschedule appointments.
- A no show fee of $50.00 will be applied to all no show appointments and must be paid prior to setting another appointment.

Prescriptions for controlled substances will not be filled during non-working hours. It is your responsibility to ensure you have refills and that you are requesting refills during business hours.
Prescriptions will not be sent to a pharmacy outside of Arizona.

If you are not having an emergency, please contact the office at 480-787-9384 by phone call or text message and we will respond within 24-48 hours during business hours.
All prescriptions will be E-Scribed directly from the office to your pharmacy. We strive to complete all refill requests as quickly as possible.
Prior Authorization Requests may take a week or more to be completed.
Tips on how to help keep things running smoothly:
** Make sure we have your current pharmacy information: address & phone number.
Make sure to update us of any changes to your insurance for prior authorizations.
Schedule your follow up appointments as directed to keep refills on a regular schedule.
Include your fullname, DOB, and pharmacy location (if applicable) when calling, emailing, or texting. This information is needed each time.
