Currently we are doing all appointments by phone and all new patient appointments by video. To fill out the new patient paperwork electronically, please contact the office by phone or text (480-787-9384) to have the link e-mailed to you.
Alternately, you may also print out the paperwork below and e-mail the completed paperwork and a copy of your ID and Insurance ID Cards to JoEllenButlerPSYNP@Gmail.com
Please keep in mind that all new patient paperwork must be completed prior to your appointment.
In order to keep our cost down and be able to provide superior service, we are strictly a self-pay provider. We do not accept insurance of any kind. However, we can provide instructions that may be helpful in getting reimbursed by your insurance provider.
- If you need prescription refills, please check with your pharmacy first.
- Ensure refill requests are being made during normal business hours and allow 24 hours for refills to be completed.
- Non-emergency refill requests made during non-business hours will be completed the following business day. (If a request is made on Friday, the request may not be completed until Monday morning). It is your responsibility to make sure that your prescriptions are filled on time and during business hours.
- We require 24 hour notice to cancel/reschedule appointments.
- A no show fee of $50.00 will be applied to all no show appointments and must be paid prior to setting another appointment.
